The timer for reacting does not reset if you find another encounter - it counts down from the last combat. This should reset every time you get to a new encounter(imho).

I noticed this and thought it was by design :)

It seems to happen with encounters very soon after the last one

No, it is not by design. The timer must not be getting reset.
